Why effective rate instead of my tax bracket?

Your bracket only taxes the last slice of income. Your effective rate — total tax divided by total income — is the real average, and it is always lower. That is the number that predicts your paycheck.

Biweekly vs. twice a month — different?

Yes. Biweekly means 26 paychecks a year (with two magical 3-paycheck months). Twice-monthly means 24. If you budget monthly, live on two paychecks and treat the third as a bonus.

Does this include Social Security and Medicare?

Not separately — this is a simplified estimate. Fold those into your effective rates, or nudge the federal rate up a few points for a closer guess.
